A Lean approach supports more collaborative primary care

Holy Family Memorial partnered with Kahler Slater to program and design a new freestanding primary care clinic, planned to serve a large rural population in central Wisconsin. Envisioned as a community wellness hub this project was inspired by the spirit of hospitality of the nearby Kohler American Club resort. Primary care services include medical imaging, a retail pharmacy and rehabilitation therapy. The rehab gym is featured prominently at the entry to the clinic, emphasized in the design as a beacon to the community.

The new clinic consolidates staff and operations from two existing clinics to improve operational efficiency. A Lean 3P event kick started the project followed by rapid prototyping and full size room mockups. Through this exploration, the design teams identified operational improvements and efficiencies to support a more collaborative relationship between provider and patient. Telehealth readiness for e-visits in any patient space and an expanding specialist network have been incorporated into the new design.

The imminent adoption of a more accessible electronic medical record provides opportunity for improvements in collaboration, care coordination, patient scheduling and room utilization. Dual access exam rooms and supporting technology provide patient self-rooming opportunities and efficiencies for a new integrated care team model. Behind the scenes contiguous exam pods are connected so that staff can flex to support varying levels of clinic activity.
